We are so excited to have your child join us for Christmas At Crossroads.
Every child (11 and under) will receive a gift this Christmas. This form will help us gather important information about your child so that we can provide a safe and enriching experience for everyone involved. And so we can choose age appropriate gifts for each child.

Crossroads Kids ages range from age 12 months- 11 years old
